Forms

Employees must select customized notice forms for each payroll company under which their payroll was processed. Employees can find this information on their paystubs or W-2. Please note that Cast & Crew maintains a policy under Massachusetts Paid Family & Medical Leave for the benefit of our clients and as part of the services we provide as a payroll employer. It is for this reason that Cast & Crew may be listed as employer on the forms and posters below. This does not alleviate clients of their employer duties under the law. Clients should ensure their own compliance as employers with respect to Massachusetts Paid Family & Medical Leave.
